In-Person Events and Surveys
When you attend an in-person continuing education event and complete our survey, we collect your name, email address, and dental license number to issue your CE certificate. Survey responses are collected anonymously and are not linked to your identity. We use aggregated survey data solely to improve the quality of our educational programs.
If you later create an account using the same email address, your in-person CE certificates will be linked to your account for your convenience.
Contact Form and HIPAA Notice
This website is not a covered entity under HIPAA and is not designed to receive, transmit, or store protected health information (PHI). Our contact form requires you to confirm that your message contains no patient information before submitting.
Please do not share patient names, dates of birth, treatment details, or any other PHI through this website.
